You will be required to email your package to The Vendor Management Team at vedorsupport@uchicago.edu. In addition, please forward the following information to the Business Diversity Department at businessdiversity@uchicago.edu:

• Copy of the supplier information form
• Copy of the certificate of compliance
• Proof of M/WBE certification (if applicable)
